Absence Request Form
Student Teachers are expected to carry out their assigned duties and responsibilities at all times, with the exception of personal illness or other circumstances beyond their control (i.e. family emergency, death in the family, etc.). Form the situations that require valid cause for requesting absence, the below form must be completed by the Student Teacher. It requires the Student Teacher to submit an electronic signature signifying that they have made all parties aware of their absence at least one week prior to the request absence. The University Supervisor has the right to require makeup work for any excused absences.
Note: Student Teachers are required to fill out the Absence Request Form per excused absence.
